Case Success Manager 個案成功經理

Case Success Manager (CSM)/ Case Success Associate (CSA) ensures case intimacy by proactivelyunderstanding the changing needs of the cases and the resource required to serve the needs. As the first point of contact for most cases, He/ She leads the outreach program at case level. He/ She also participates in providing service providers training for compliance and service quality improvement purposes. 個案成功經理 (CSM) 個案成功專員（CSA）透過主動了解個案的變化需求和滿足這些需求所需的資源，確保與個案之間的親密關係。作為第一聯絡人，他/她負責在個案層面上領導外展計劃。他/她還參與提供服務提供者的培訓，以達到合規和提升服務質量的目的。

Responsibilities/ 職責

• Outreach program - case-level (e.g. phone and visit potential cases, respond to all home based service related inquiries) 外展計劃 - 個案層面（例如，致電和拜訪潛在個案，回應所有與上門照顧服務有關的查詢）
• Case management (e.g. arrange periodic visits to current cases) to refine service offerings based on the changing needs 個案管理（例如，定期安排拜訪現有個案）以根據不斷變化的需求完善服務提供。
• Participate in training to service providers 參與服務提供者的培訓
• Constantly seek ways to improve service quality and operations efficency 不斷尋求提升服務質量和運營效率的方法
• Other tasks assigned by Case Success Director 個案成功總監分派的其他任務

Requirements / 職位要求

• Excellent customer-first mentality 以客戶為先的態度。 優秀的聆聽能力
• Excellent commands on instant messaging app (Whatsapp, Wechat) and ability to learn new app 即時通訊應用程式（如WhatsApp、微信）操作能力，並具備學習新應用程式的能力
• Good commands on MS office and Google Suite products 良好的Microsoft Office和Google Suite產品操作能力
• 良好的Microsoft Office和Google Suite產品操作能力
• Holder of Associate Degree or above. Professional Qualification holders (RSW, EN/ RN, Physiotherapist, Occupational Therapist) are welcomed. 持有副學士或以上學歷.持有專業資格（註冊社工、登記/註冊護士、物理/職業治療師）亦歡迎申請。
